The most likely culprit is the primary sensor.
Does the boiler not display any fault codes or flashing lights.
If the boiler has a divertor valve then the diaphragm might be stretched and needing replaced.

I am assuming you are in the UK, and that you are using this for both heat and hot water?
Typically, this is caused by a faulty flow-switch/diverter valve. This valve may be sticking due to sludge or age. Could also be the secondary (domestic water) heat exchanger is clogged.
Get it serviced. Demand-type water heaters should only be touched by competent and qualified technicians.
